Forms for the Student Affairs Office

Add the statutory representative to the header of the agreement (see ARES database or www.justice.cz in the ARES database), or have the power of attorney for a signature, possibly a mandate or an internal standard, to ensure that the signatory is a person authorized to do so).
Add name, supervisor position and contact details.
Enter the name and surname (including titles), telephone number and email address of the contact person (the "guarantor of internship") in the header of the UP information.
Add information about the student to the header of the agreement.

In Article I of the Agreement (Object of Commitment), fill in the place (name of the institution and department), time and extent of the professional internship and information about the participating student.

In Article II of the Agreement (Rights and obligations of internship provider) include the name and surname (including titles), telephone number and e-mail address of the employee supervising the student during the professional internship.

Signature page – Internship Provider:

Statutory representative of the internship provider must be verified in the ARES database or www.justice.cz. Add the statutory representative to the header of the agreement (see ARES database or www.justice.cz in the ARES database), or have the power of attorney for a signature, possibly a mandate or an internal standard, to ensure that the signatory is a person authorized to do so).
In printed characters, under the signature line, state the name and surname of the statutory representative of the internship provider (including titles) and his/her function (similar to the signature of the Dean).

Signature page – Student:

Under the signature line list the name and surname of the student.

Organizational instructions for the signing of a agreement to ensure professional internship of a FoS student:

The content of the agreement must be checked by the guarantor (or instructor) of the subject (hereinafter referred to as the "guarantor"), within which the internship is executed. The name of the subject's guarantor is stated in IS/STAG. Based on the verification and initiation of the agreement by the guarantor, the agreement may be sent to the internship provider and then by the student. The Dean is the last one to sign the agreement; the agreement is submitted in 3 copies for the Dean's signature, exclusively through the Dean's Secretariat.
All three original copies of the agreement signed by all parties are returned by the Dean's Secretariat to the internship guarantor at the department (who will ensure that the original copies are forwarded to the internship provider, the student and the department for archiving).
